Visual activity schedules are among the most extensively studied visual supports for autistic individuals. A comprehensive review of 31 peer-reviewed single-case studies (Knight, Sartini, & Spriggs, 2015) evaluated the quality of the visual activity schedule literature using rigorous evidence-based practice criteria. The review concluded that visual activity schedules meet criteria for an established evidence-based practice, demonstrating positive effects across play skills, daily living skills, and academic tasks from preschool through adulthood.
Visual schedules are also recognized as an established intervention in the National Standards Project: Phase 2 (2015) for increasing independence and helping individuals plan for upcoming events. A broad third-generation review of evidence-based practices for autistic children and youth (Hume et al., 2021) provided further confirmation of visual supports as a recognized practice.
Research indicates that visual schedules can reduce prompt dependency, support independent task completion, and help decrease challenging behaviors associated with unexpected activity changes. They capitalize on visual processing strengths frequently observed in autistic individuals and reduce reliance on verbal instruction, which may be more difficult to process for some children.
Honest Limitations
A 2025 systematic review (Mouzakes) found that while visual schedules are consistently used in transition-related interventions, existing studies rarely isolate the visual schedule as the active component. Most studies combine visual schedules with other documented practices such as reinforcement and prompting, making it difficult to attribute outcomes to the schedule alone. Component analysis research in this area remains limited.
Research also indicates that visual schedules alone may be insufficient to reduce problem behavior during transitions from preferred to non-preferred activities. They are most effective as part of a broader structured approach that may include advance warnings, reinforcement strategies, and environmental supports.

2. Choice Board

What the Research Shows
Providing structured choice-making opportunities is a well-supported strategy for increasing on-task behavior and decreasing problem behavior in autistic children. A growing body of peer-reviewed research demonstrates that when children are given the opportunity to choose between activities, materials, or reinforcers, engagement increases and challenging behaviors decrease.
A study by Ulke-Kurkcuoglu and Kircaali-Iftar (2010) comparing activity choice and material choice in children with autism spectrum disorders found that on-task behavior increased under both choice conditions compared to no-choice conditions, suggesting that choice opportunities may be motivating for some learners. Choice making is recognized as an antecedent-based intervention in autism support practice.
When children can make meaningful choices within their routine, they develop a sense of control that supports emotional regulation, reduces anxiety, and increases willingness to engage in less preferred activities. For transitions specifically, offering limited choices about how an activity ends or what comes next can reduce resistance and support smoother movement between tasks.
Visual choice boards extend the benefit of choice-making by reducing language comprehension demands, allowing children to indicate preferences through pointing or indicating rather than verbal response alone.
Implementation Guidance from the Literature

Honest Limitations
Most choice-making research involves structured one-on-one or academic settings. Evidence for direct application to daily home-based transitions, while theoretically consistent, is more limited than the classroom-based literature. Effectiveness depends on meaningful, appropriate options that align with the child's interests and preferences, requiring some degree of individualization.

3. Sand Timer (Visual Time Representation)

What the Research Shows
Many autistic children experience difficulty with time perception, which contributes to challenges during transitions. Research identifies key time-related challenges: difficulty understanding time concepts and reading clocks, difficulty regulating behavior in relation to time, and difficulty anticipating how long activities will last.
Visual timers address these challenges by making time concrete and observable. Rather than relying on abstract verbal warnings, a visual timer provides an immediate, unambiguous signal about how much time remains in an activity. This supports the predictability that many autistic children need to prepare emotionally for transitions.
The most-cited study in this area (Dettmer et al., 2000) examined three visual transition supports, including a visual timer, for two children with autism. Noncompliance during transitions decreased when visual supports were used alongside instructions. This study used a clock-style visual timer (Time Timer), not a sand timer specifically, and outcomes reflect a package of multiple visual supports rather than any single tool in isolation.
Sand timers function similarly to clock-style visual timers in that they make the passage of time observable. No peer-reviewed studies to the authors' knowledge have examined sand timers specifically as a transition support. Their inclusion in this kit is supported by the broader evidence base for visual time representation and advance-warning strategies.
A 1995 study by Tustin found that providing advance notice of an upcoming task change decreased transition-related stereotypy in a participant with autism, demonstrating the value of any structured, predictable signal that an activity will end soon.
Honest Limitations
No peer-reviewed research specifically examines sand timers as a standalone transition support. The evidence for this section is drawn from the broader visual time representation literature, which uses clock-style timers in multi-component intervention designs. Sand timers are included based on their functional equivalence to other visual timers rather than direct empirical testing.

4. Wordless Visual Storybook

What the Research Shows
Wordless picture books occupy an established place in autism research, most commonly as tools for narrative elicitation, social cognition assessment, and communication evaluation. They have been used in numerous peer-reviewed studies with autistic children to examine narrative skills, theory of mind, and visual attention, which reflects their utility as accessible, language-reduced tools for this population.
Research on picture books broadly supports their value for autistic children. Studies have found that the predictable, structured narrative format of picture books helps reinforce desired behaviors, provides a sense of security, and reduces anxiety. A 2025 eye-tracking study (Zhang et al., Scientific Reports) examining how autistic preschoolers engaged with picture books found meaningful patterns of visual attention, suggesting picture books are actively processed by this population.
The specific benefit of the wordless format is that it removes barriers created by text decoding or expressive language demands, allowing children who benefit from visual processing to engage with narrative independently. The use of a consistent character across scenarios, as designed in CorbsWorld, aligns with research suggesting that predictable, repeated visual elements support comprehension and reduce cognitive load.
Honest Limitations
No peer-reviewed studies to the authors' knowledge have directly evaluated wordless picture books as a standalone tool specifically for reducing transition-related distress. The evidence base supports the use of picture books and visual narrative tools broadly for autistic children; research specifically on wordless formats as a transition support is not yet available in the peer-reviewed literature.

5. Noise-Reducing Headphones
Evidence Level: MODERATE
What the Research Shows
Noise-reducing headphones help reduce background noise so children can focus, relax, or enjoy quiet activities. They are passive noise-reduction earmuffs — requiring no electronics, batteries, or audio connection — and published attenuation and noise reduction ratings for passive earmuffs typically range from approximately 20 to 27 dB (varies by model; Pfeiffer et al., 2019). Auditory hypersensitivity is among the most commonly reported sensory differences in autism. Research estimates that a large proportion of autistic children exhibit hypersensitivity to auditory stimuli (Pfeiffer et al., 2019), and neurophysiological studies have documented prolonged auditory cortex responses and higher physiological arousal at baseline in this population. This sensory difference is directly relevant to transitions, where environmental noise may heighten stress and reduce a child’s capacity to manage behavioral demands.
A 2019 pilot study (Pfeiffer et al.) evaluated noise-attenuating headphones in six children with autism and confirmed auditory hypersensitivity. The study reported decreased electrodermal indices from baseline and concluded that headphones may reduce sympathetic activation in natural settings. A qualitative follow-up study found that parents and teachers reported increased participation in home, community, and school settings when children used noise-attenuating headphones.
A 2024 study (Zanin, Tomlin, & Rance) found that noise-reducing earbuds significantly improved auditory attention and classroom listening behaviors in children with autism, though speech perception in noise was not significantly improved. A 2016 pilot study (Ikuta et al.) found that earmuffs showed statistically significant improvement in goal-attainment outcomes for 21 children with autism and auditory hyperreactivity, while noise-reducing headphones showed improvement in a subset of participants.
Honest Limitations
Studies in this area are predominantly small pilot studies and single-case designs. A 2021 systematic scoping review found that, while clinical recommendations to use noise-reducing headphones are widespread, none of the recommendations reviewed cited empirical evidence directly.
Headphones may not be suitable or comfortable for all children, as some may find the pressure or feel of headphones aversive. Individual fit, preference, and tolerance vary significantly. Consistent headphone use may also limit communication awareness and social engagement in shared environments and should be used thoughtfully within routines.

6. Weighted Blanket

What the Research Shows
Weighted blankets are widely used as a sensory support for autistic children, and the theoretical rationale is grounded in deep pressure stimulation and its proposed calming effects on the autonomic nervous system. Some caregivers and practitioners report subjective benefits including increased comfort, improved willingness to settle into rest routines, and a perceived sense of calm.
Laboratory research on deep pressure stimulation has demonstrated possible effects on physiological arousal markers, providing a neurophysiological rationale for why weighted items might support regulation. This theoretical grounding is often cited as more established than the clinical evidence for weighted blankets specifically, though that characterization reflects an interpretive judgment about relative evidence strength.
Honest Limitations
The largest and most rigorous clinical trial on weighted blankets in autistic children is a randomized, placebo-controlled crossover trial (Gringras et al., 2014, Pediatrics) involving 73 children ages 5–16 with confirmed autism and severe sleep problems. This study found that weighted blankets were not significantly more effective than a standard unweighted blanket in improving total sleep time, as measured by actigraphy.
Smaller single-subject design studies (Gee et al., 2016) have reported minimal changes in objective sleep outcomes, with authors noting findings were insufficient to recommend weighted blankets for clinical use based on objective data alone.
A 2024 narrative review (Frontiers in Psychiatry) reported mixed-to-positive findings across populations and concluded that more high-quality large-scale randomized trials are needed to validate efficacy. The overall evidence base may be considered limited in the absence of such trials, though the review itself noted some positive subjective wellbeing outcomes.

7. Calming Stone (Tactile Sensory Object)

What the Research Shows
Tactile sensory objects are grounded in sensory integration theory and the well-documented role of proprioceptive and tactile input in supporting self-regulation. Autistic individuals frequently exhibit sensory-seeking behaviors, including tactile seeking, as a means of modulating arousal and reducing anxiety. Providing a structured, portable outlet for this input is a commonly recommended strategy in occupational therapy practice.
A 2024 peer-reviewed study (Roche, Back, & Van Herwegen) using parent-report measures found that parents of autistic children rated fidget toys as more effective for anxiety reduction than did parents of neurotypical children, suggesting that sensory-seeking behaviors in autistic children may be particularly responsive to tactile input. A parent survey found that children with higher sensory-seeking behaviors benefited most from fidget tools.
Honest Limitations
A 2025 peer-reviewed study (Hixson & Radley, Contemporary School Psychology) examined the effects of fidget devices on disruptive behavior in middle school students with autism using a single-case alternating treatment design. The study found inconclusive effects on behavioral patterns across all three devices tested and noted the need for more rigorous research before strong efficacy claims can be made.
Most studies on tactile sensory tools rely on caregiver report rather than objective behavioral measurement, sample sizes are small, and individual variability in sensory preferences is high. What is calming for one child may be distracting or aversive for another.
The calming stone is included as an individually variable sensory support informed by occupational therapy practice and caregiver experience. It is not positioned as a clinically proven intervention, and its usefulness will depend on the individual child's sensory profile, preference, and context of use. Occupational therapy consultation is recommended when sensory regulation is a significant area of concern.
